Cardiovascular diseases, malignant tumors, and cerebrovascular diseases are the three major
diseases that threaten human health. Among them, cardiovascular disease is considered “the
number one killer”. Due to its rapid changing in condition and high risk of mortality, attending
physicians in cardiovascular medicine face a large volume of emergency consultations and routine
consultations daily. In the consultation cases, there are patients with other medical conditions
combined with cardiovascular diseases, surgical patients requiring preoperative cardiovascular
risk assessment, obstetrics and gynecology patients with concomitant cardiovascular diseases
requiring preoperative or delivery risk assessment, and patients with heart failure caused by
various reasons (such as chronic atrial fibrillation). Cases like coronary artery stenting patients
with gastrointestinal bleeding, cerebral hemorrhage, or the need for non-cardiac surgery
involve consultations on cardiac function assessment, anticoagulation, and antiplatelet therapy
management. These cases have a high level of specialization, and the attending physicians may
not fully explain the situation when consulting other departments, leading to repeated inquiries to
senior physicians or even repeated consultations from clinical departments. For these reasons, we
conceived the idea of inviting experienced clinicians who have been working on the front lines
to compile a book entitled Cardiovascular Consultation Summary to address common difficulties
encountered during clinical consultations, thereby meeting the needs of both the consulting
departments and the cardiovascular physicians providing consultations. Inevitably, due to our
limited capabilities, there may be oversights and shortcomings, and we would like to welcome
readers’ critiques and suggestions.
This book is divided into seven chapters that introduce the most common questions faced by
cardiovascular physicians during consultations. Chapter 1 covers coronary artery disease, which
has the highest incidence and broadest scope among critical cardiovascular diseases, including
differential diagnosis of acute chest pain, treatment procedures for ACS, diagnosis and treatment
of sudden death, risk stratification, treatment strategies, management of complications, and
medication management. Chapter 2 discusses the management of heart failure, which is the “final
battleground” of cardiovascular diseases, with not only a high incidence but also complex clinical
situations for some patients. In addition to introducing basic knowledge about heart failure,
this book also includes the differential diagnosis of dyspnea and edema, which are frequently
encountered during consultations. Chapter 3 covers arrhythmias, a challenging topic that most
physicians are hesitant to address. From the perspective of identifying and providing emergency
treatment for various high-risk arrhythmias, this book aims to help less experienced physicians
quickly grasp the principles of rapid identification and management. Chapters 4 and 5 discuss

various clinical situations related to hypertension, pericardial diseases, and valvular diseases,
including refractory hypertension, secondary hypertension, and pulmonary hypertension, which
are frequently encountered during consultations. Chapter 6 introduces the management principles
for various cardiovascular diseases during pregnancy. With the implementation of the two-
child policy and the increase in women’s educational level, the number of older and medically
complicated pregnancies has surged, necessitating greater knowledge in this area for consulting
physicians. Chapter 7 covers cardiovascular risk assessment for non-cardiac surgeries, which is
a significant consultation request from surgical departments. This book discusses preoperative
assessment, surgical risk classification, perioperative cardiovascular evaluation methods, the
management of concomitant cardiovascular diseases, and perioperative examinations.
We believe that the issues faced by cardiovascular physicians during consultations are not
limited to the above content. This book aims to familiarize consulting physicians with the most
common clinical scenarios and effective management strategies to meet the majority of clinical
needs.
